Friday Night Team Registration

Tournament Headquarters:

All teams are required to register their teams Friday, December 2, 2011 from 5:00 p.m. - 9:00 p.m. Virginia Beach Field House, 2157 Landstown Road, Virginia Beach, VA, Phone: (757) 427-3955. Teams needing special arrangements for registration (Saturday) should email tournaments@olddominionsc.org no later than Wednesday, November 30th.  Credentials will be checked (player cards, official roster, medical releases, permission to travel if required, etc.).  
 
Please be prepared to bring the following to registration:
  1. Original and 2 copies of your state-approved roster (if you mailed a copy with your application you will need only one copy of your state-approved roster).
  2. USYSA player passes for each player, including guest players.  Pictures must be permanently attached.
  3. A signed medical release form (your local club’s medical release forms will be acceptable) for each player including the guest players.  The medical release forms do not have to be notarized.
  4. Permission to travel - for teams outside of Region 1 and for New Jersey and Connecticut teams - who are not members of US Club Soccer.

Credentials must be available at the tournament fields if requested by tournament staff or referees. 

Team Contact Information:
Each team, upon checking in at registration, must sign a ledger sheet giving the hotel where the team is staying, the room number of a team contact, the team's coach or the team's assistant coach so that these individuals may be contacted if needed.
